Short Term Disability

ARUP Laboratories offers company paid Short Term Disability to all benefit eligible employees. The steps are as follows:

  1. Lincoln Absence Management will start the short term disability process when you contact them to file for a continuous leave of absence for your own medical condition.
  2. Once approved by Lincoln, you could receive 60% of your weekly salary
  3. Lincoln deposits short term disability payments directly into your bank account
  4. ARUP will supplement approved leaves with 40% grandfathered LTS (if there is a balance in your LTS bank)
  • Approved disability benefits have a 7 day waiting period, unless hospitalized
  • Waiting period is waived if hospital admission occurs
  • The premium that ARUP pays for you will be taxed
  • The pay that you receive under short term disability is tax free
  • If you don’t have grandfathered LTS, you can choose to use PTO or go unpaid

Definitions
  • Eligibility: All active employees who are scheduled 20 hours or more per week, beginning the first of the month after you have worked 30 days.
  • Benefit: 60% of your weekly salary – to the max of $2500
  • Disability: Covered injury or illness that restricts an employee from performing the essentials functions of their job
  • Covered Earnings: Wages or salary and commissions, not including bonuses and other extra compensation
  • Elimination Period: approved disability has a 7 day elimination period before benefits are payable; if hospitalization occurs the elimination period is waived.
  • Benefit Duration: benefits are payable during the approved time period that has been certified by your treating physician and Lincoln up to a maximum of 90 days.

Long Term Disability

Should your medical condition keep you out of work for longer than 90 days, ARUP will help you file a long term disability claim. Long term disability insurance is provided for all employees who work 20 hours or more each week.

Lincoln Absence Management will send you an application packet in the mail when you are nearing exhaustion of short term disability benefits. You will need to complete and return the packet to Lincoln to be considered for long term disability.

  • Permanent or temporary disability
  • Once approved by Lincoln, you could receive 60% of your salary for the maximum benefit period
  • You must be continuously disabled for 90 days before benefits may be payable
  • Lincoln deposits long term disability payments directly into your bank account
  • The premium that ARUP pays for you will be taxed

  • Benefit: 60% of your monthly salary – to the max of $10,000
  • Disability: You are unable to perform the essential functions of your job solely because of a covered injury or illness. Also, you are unable to earn 80% or more of your indexed earnings from working in your regular occupation.
  • Covered Earnings: Wages or salary and commissions, not including bonuses and other extra compensation. Bonuses and other compensation may be covered under an individual policy.
  • Elimination Period: You must be continuously disabled for 90 days before benefits may be payable
  • Benefit Duration: Should you remain disabled, approved benefits may continue until retirement age or you no longer qualify for benefits, whichever comes first.

Sick Pay

  • Full-time and part-time employees are eligible for sick pay as of August 6, 2022.
  • These hours can be used to recover from mental or physical illness or injury, or to help care for immediate family members who are ill or injured.
  • Employees can check the status of their balance in Kronos.
Requesting Sick Pay
  • Submit requests for sick pay in Kronos just as you would submit other requests for time off. Make sure the sick pay option is selected.
  • If you need to leave work due to an illness, submit your request for sick pay when you return to work.

*The 7on/7off schedule is qualified to receive up to 1.15 hours per pay period with 30 hours as the maximum accrued.
What You Need to Know
  • Sick pay will accrue each pay period based on the number of hours worked during that period.
  • Sick pay is used in full-day increments unless you become ill during your shift.
  • If you are out of work due to a serious health condition, file for a Leave of Absence with Lincoln Financial Group.

Grandfathered Long Term Sick

  • Grandfathered LTS was accrued by employees prior to 2019
  • Use only for an employee’s own medical condition.
  • Check in Kronos to see your balance
How to use GLTS if sick for a few days

If you have been out of workfor a few days,due to a non-serious, acute health condition*, please follow these three steps:

  1. Fill out a Request for Long-Term Sick form
  2. Attach ahealth care provider’snote
  3. Submit both the form and note to HR or absence@aruplab.com

*Examples of non-serious, acute health condition: colds/flu, headaches (other than migraines), upset stomachs/minor ulcers, earaches, routine dental or orthodontic problems, etc.

What you need to know:

  • You cannot submit a request for long term sick to HR, if you are missing work due to surgery or a serious health condition. In these cases you will need to file for FMLA or MLOA
  • PTO will be used for the first 2 full shifts and if you have any grandfathered LTS, it will be used beginning on the 3rdday missed. If you do not have PTO, the first 2 full shifts will go unpaid.
  • If you don’t have any grandfathered LTS, please work with your supervisor to enter PTO instead of completing the grandfathered LTS form.
How to use GLTS on a leave of absence

You can use any accrued grandfathered long term sick for the unpaid portion of an approved leave of absence for your own medical condition.

Once your leave has been approved by Lincoln, HR will automatically enter any accrued GLTS into your time card as follows:

100% GLTS = 7 day waiting period before short term disability begins

40% GLTS = to supplement 60% short term disability payments from Lincoln

What you need to know
  • Bonding time does not qualify for the use of GLTS
  • If you are out of work for a few days due to a non-serious, acute health condition, you would need to complete a request for grandfathered LTS form using the process above.
  • If you do not have grandfathered LTS, you can choose to use PTO or go unpaid

Paid Parental Leave (PPL)

ARUP recognizes that the time immediately following the birth or adoption of a child is crucial for family members to bond and adjust to new circumstances. We hope this new benefit will ease the transition for employees and their families

  • Birth or adoption of a child that takes place after January 1, 2021 may qualify
  • Offered to PT and FT employees that have been employed at least 1 year
  • Runs concurrent with a Bonding Leave of Absence (FMLA or Personal)
  • Up to 120 hours PPL at 100% of your base pay
What is Paid Parental Leave (PPL)

If eligible, you are able to take up to 120 hours of PPL to bond with the new addition to your family and you will receive 100% of your base pay during that timeframe.

Those employees who work in a state with a similar leave paid by the state, ARUP will make up the difference with up to 100% of your base

  • Eligibility: PT and FT employees that have been employed at least 1 year
  • You must use PPL within 90 days of bonding eligibility
    • Mothers – bonding starts after disability portion of leave (6 or 8 weeks)
    • Secondary Parent – bonding starts at birth
    • Adoption – bonding starts at adoption
  • PPL can be taken in one or two blocks of time.
  • Hours are prorated based on hours scheduled per week.

How to Request Paid Parental Leave (PPL)

In order to receive this benefit, you must apply for PPL within 30 days of the child’s birth or adoption by completing the steps below.

  1. You will need to set up bonding leave with Lincoln Financial that will run concurrently with PPL.
    • To begin a request for FMLA or Personal leave contact Lincoln Absence Management by phone:
      • Call 1-888-244-7535
      • Option 1 (you are an insured)
      • Enter your Social Security Number
      • After confirming social security number, select option 1, which will connect you to a representative that will assist with both the absence and short term disability claim.
  2. You will also need to complete the documentation to access pay.
