You can access and review your Explanation of Benefits (EOB) though MyChart, your secure, online connection to your health information. To activate your account or request paper statements contact Member Services at 801-213-4008 or toll free 833-981-0213.
Once in MyChart go to, Health plan, coverage details, select yourself or one of your dependents, then click on Claims.
