Specimen Required: Patient Prep:  Supine:  1.  Sample obtained 8-9 a.m.; 2. Fasting starting at midnight; 3. Normal sodium diet (100-200 mEq/day) for at least three days.; 4.  Receiving no medications known to affect renin-aldosterone system. Upright: 1.  Samples obtained before noon (after at least two hours in upright position),; 2.  Normal sodium diet (100-200 mEq/day) for at least three days.; 3. Receiving no medications known to affect renin-aldosterone system.    Contact Medical Director for more information.

Collect:  One 6 mL plastic serum separator tube and one 5 mL lavender (EDTA) or pink (K2EDTA).

Storage/Transport Temperature:  1 mL serum and 1 mL plasma  (Min: 0.5 mL serum and 0.6 mL plasma), frozen.  Submit specimens in an ARUP Standard Transport Tube.

Remarks:  CRITICAL FROZEN.  Separate specimens must be submitted when additional tests are ordered. Do not refrigerate.  Do not collect in refrigerated tubes.  Separate plasma from cells ASAP and freeze.

Unacceptable Conditions:  Refrigerated plasma.

Stability (collection to initiation of testing):  After separation from cells: Ambient: 4 hours; Refrigerated: Unacceptable; Frozen: 1 month

An Aldosterone/Renin Activity Ratio of greater than 25 is suggestive of hyperaldosteronism.
