Ordering Recommendation: Use in IgA-deficient individuals; acceptable single screening test for celiac disease. Acceptable follow-up test for weakly positive or negative tissue transglutaminase IgG screen. Celiac Disease Reflexive Panel (0051065) is the preferred screening test for individuals with high suspicion for IgA deficiency or patients at-risk for celiac disease who should undergo small bowel biopsy to confirm disease.

Specimen Required: Collect: Serum separator tube.

Specimen Preparation: Separate serum from cells ASAP or within 2 hours of collection. Transfer 1 mL serum to an ARUP Standard Transport Tube. (Min: 0.15 mL)

Storage/Transport Temperature: Refrigerated.

Unacceptable Conditions: Contaminated specimens.

Stability (collection to initiation of testing): After separation from cells: Ambient: 48 hours; Refrigerated: 2 weeks; Frozen: 1 year (avoid repeated freeze/thaw cycles)

Interpretive Data: The presence of endomysial antibodies IgG, may be useful in identifying IgA-deficient patients at risk for celiac disease. To establish the diagnosis of celiac disease, a positive result must be confirmed through a biopsy of the small intestine.
