Specimen Required: Collect: Tissue.

Specimen Preparation: Formalin fix (10% neutral buffered formalin) and paraffin embed tissue. Transport representative tissue block or 10 representative sections on "plus" (positively) charged or silanized slides. (Min: 5 slides) Protect from excessive heat.

Storage/Transport Temperature: Room temperature. Ship in cooled container during summer months.

Unacceptable Conditions: Tissue fixed in fixative other than 10% neutral buffered formalin.

Stability (collection to initiation of testing): Ambient: Indefinitely; Refrigerated: Indefinitely; Frozen: Unacceptable
