[ image for: Time Sensitive]
Time Sensitive

Test Mnemonic: NEUT RECEP
Methodology: Flow Cytometry

Performed: Sun-Sat

Reported: 1-3 days

Specimen Required:
Collect: One 7 mL green (sodium or lithium heparin) from patient AND one 7 mL green (sodium heparin) from normal control subject drawn at the same time.  Client must supply the normal control.

Transport: 5 mL whole blood from patient AND 5 mL whole blood from control subject drawn at the same time, at 2-8°C.

Pediatric Collection/Transport: 1 mL patient whole blood and 1 mL control whole blood at 2-8°C.

Remarks: CRITICAL REFRIGERATED. Sample must be received within 24 hours of collection.  Antigen densities cannot be reported unless a normal control sample is also supplied.

Unacceptable Conditions: Clotted, ambient, or frozen samples. Samples submitted without a normal control drawn at the same time. Samples greater than 24 hours.

Stability: Ambient: Unacceptable; Refrigerated: 24 hours; Frozen: Unacceptable

Reference Interval:

Component Reference Interval
% CD11b 90-100
CD11b Density 50-200
% CD15 98-100
CD15 Density 55-183
% CD16 90-100

% CD18 90-100
CD18 Density 60-190

Interpretive Data:
The Neutrophil Receptor Panel measures CD11b, CD15, CD16, and CD18 on neutrophils. CD11b is the receptor for the C3bi fragment of complement. CD15 is a ligand for E and P selectins.  CD16 is the IgG Fc receptor. CD11b is an alpha subunit, and CD18 is the beta subunit of Mac-1, also known as CR3. All of these receptors are normally found on neutrophils. The percentage of patient neutrophils bearing these receptors is reported. In addition, the receptor surface density of each antigen is compared to normal neutrophils and expressed as a percent. Values outside of the reference interval may correlate with abnormal neutrophil function. For example, CD18 is decreased or absent in leukocyte adhesion deficiency (LAD) type I and CD15 is decreased or absent in LAD type II.
