Mnemonic: HSV BLOT

Methodology: Immunoblot
Performed: Mon
Reported: 1-8 days
Specimen Required: Collect:  One 4 mL serum separator tube.

Transport:  1 mL serum at 2-8°C. (Min: 0.2 mL)  Submit specimen in an ARUP Standard Transport Tube.

Pediatric Collection/Transport:  0.1 mL serum at 2-8°C.

Remarks:  Separate serum from cells ASAP. Acute and convalescent specimens must be labeled as such; parallel testing is preferred and convalescent specimens must be received within 30 days from receipt of acute specimens.  Please mark specimen plainly as "acute" or "convalescent."

Unacceptable Conditions:  Hyperlipemic, heat-inactivated, hemolyzed, or contaminated specimens.

Stability:  After separation from cells: Ambient: 2 days; Refrigerated: 2 weeks; Frozen: 1 year (avoid repeated freeze/thaw cycles)
Reference Interval:
By report
Interpretive Data: Individuals infected with HSV may not exhibit detectable IgG antibody to type specific HSV antigens 1 and 2 in the early stages of infection. Detection of antibody presence in these cases may only be possible using a nontype-specific screening test.  All results from this and other serologies must be correlated with clinical history, epidemiological data, and other data available to the attending physician in making the differentiation between HSV type 1 and HSV type 2.

Patient Interpretation Band Reactivity

HSV-1 and HSV-2 positive.  Presumptive evidence of HSV-1 and HSV-2 exposure. + + +
HSV equivocal. A second sample should be drawn in two to four weeks and retested by Immunoblot or other type-specific HSV IgG method. + Neg Neg
Negative. No evidence of HSV exposure detected. Neg Neg/+ Neg/+

Note: Type-specific ELISA antibody tests for herpes simplex virus types 1 and 2 antibody, IgG are now available which use the same cloned antigens as in the immunoblot.  Sensitivity and specificity for both the type-specific ELISA and immunoblot are similar.  The ELISA assay is offered Sunday through Saturday.  Refer to Herpes Simplex Virus Type 1 Glycoprotein G-Specific Antibody, IgG by ELISA (HerpeSelect®) (0050292) and Herpes Simplex Virus Type 2 Glycoprotein G-Specific Antibody, IgG by ELISA (HerpeSelect®) (0050294).
CPT Code(s): 86695 Herpes simplex type 1; 86696 Herpes simplex type 2
