ARUP's Laboratory Test Directory

0049015: Osmotic Fragility, Erythrocyte

Test Mnemonic: OSM FRAG
Methodology: Spectrophotometry

Performed: Mon-Fri

Reported: 1-5 days

Specimen Required:
Collect: One 5 mL green (sodium or lithium heparin).

Transport: 5 mL whole blood and two unfixed smears. Transport at 20-25°C. Send Sunday-Thursday only.

Pediatric Collection/Transport: 1 mL whole blood and two unfixed smears. Transport at 20-25°C.  Send Sunday-Thursday only.

Remarks: Samples must be received within 24 hours of collection; testing must be performed within 48 hours of collection.

Unacceptable Conditions: Hemolyzed blood.

Stability: Ambient: 24 hours

Reference Interval:
Within normal curve limits. A graph will accompany laboratory report.

Interpretive Data:
For patients with acute hemolysis, a normal red cell osmotic fragility test result cannot exclude an osmotic fragility abnormality since the osmotically labile cells may be hemolyzed and not present. Recommend testing during a state of prolonged homeostasis with stable hematocrit.
