ARUP Ordering Information
ARUP clients include many of the nation’s university teaching hospitals and children’s hospitals, as well as multihospital groups, major commercial laboratories, group purchasing organizations, military and other government facilities, and major clinics. Healthcare providers who are interested in sending testing to ARUP should check with their local hospital or reference laboratory; for more information, please contact an ARUP genetic counselor.
ARUP Test-Request Forms
ARUP test-request forms are specific to each client laboratory or hospital and are thus not available on the ARUP website. However, they are available through the client hospital/laboratory sendout department.
ARUP Patient History Forms
ARUP patient history forms are required for ARUP to perform most genetic testing and/or interpret patient results. They can also be used to help specify which test to order when communicating with the ordering laboratory. See the Patient History and Consent Forms page for a list of disorders with their associated forms. These forms should accompany each genetic test order. The Laboratory Test Directory has descriptions of the specific tests available for each genetic disorder or class of disorders. ARUP Test Prices and Billing
Test prices are not available online as billing is determined by the lab or hospital ordering the test. For pricing, please contact your institution’s billing department and provide the ARUP test code you are inquiring about. ARUP bill the client laboratory directly; we do not bill patients or third-party payers for services.
