Personal Time Off
(PTO)

PTO is accrued according to the following schedule:

Employment Vacation
0–1 years 17 days/year
1–2 years 18 days/year
2–3 years 19 days/year
3–4 years 20 days/year
4–5 years 21 days/year
5–9 years 22 days/year
10–14 years 26 days/year
15–19 years 28 days/year
20 and over 30 days/year

Upon regular employment of 30 or more hours. PTO is accrued each paycheck based on hours paid for the pay period. (Employees working a 7-on/7-off schedule do not accrue PTO; however, they receive 10 hours of premium pay each pay period.)

Long-Term Sick (LTS)

LTS is accrued at 5.33 hours per month to a maximum of 90 days.

Same as PTO eligibility.

Paid Holidays

ARUP designates eight paid holidays:

Human Rights Day Labor Day
President's Day Thanksgiving Day
Memorial Day Christmas Day

Upon regular employment of 30 or more hours per week.  (Not available to employees working a 7-on/7-off schedule.)

Funeral Leave

Provides the employee’s normal base pay to attend the funeral or memorial services of an immediate family member.

Upon regular employment of 20 or more hours per week.

Leave of Absence

Extended time off for illness, injury, family, military duty, or personal reasons.

May be available depending on circumstances.

Adoption Benefits

Up to $2,000 annually, per family, based on 80 percent of the usual and customary fees for legal adoptions of children under age 18.

After 12 months of regular employment at 30 or more hours per week.

Tuition Reimbursement

Reimbursement up to 75 percent of eligible tuition expenses for  employees working 30 or more hours per week.  Part-time  (20–29 hrs. per week) employees are eligible for 50 percent reimbursement.  MLS, CLS, and Histotechnician programs: 100 percent.  Spouse and dependent benefits: 50 percent.

After 6 months of regular employment.  After two years of service this benefit is extended to employee’s spouse and dependents at 50 percent.

Health Benefits*

Optional hospitalization, major medical, and prescription services.

Upon regular employment of 20 or more hours per week.

Employee Health Clinic

Routine screening and basic healthcare for employee and dependents including: well baby; child and adult checkups; immunizations; minor surgical procedures; illness diagnosis and treatment; prescriptions; referrals; free laboratory testing, etc.

Upon regular employment with ARUP.  Spouse, registered domestic partner, and dependents are also eligible.

Health and Wellness Program

This benefit consists of exercise, weight management, healthy behavior, smoking cessation, stress management, and self-care programs; on-site group fitness; CPR and first-aid classes; and health fairs.

Upon regular employment with ARUP.

Employee Assistance Program (EAP)

Professional short-term counseling for: stress, parenting, abuse, aging, drugs/alcohol, grief, marriage, work, finances, depression, relationships, eating disorders, and legal matters.

Upon regular employment with ARUP.  Available to everyone living under employee’s roof.

Dental Insurance Plan*

Optional preventive, basic, and major services.

Upon regular employment of 20 or more hours per week.

Vision Service Plan*

Optional benefit provides eye examinations annually, lenses or contacts every 12 months, and frames every 24 months.

Upon regular employment of 20 or more hours per week.

Group Life Insurance

Term life insurance, accidental death insurance, and Assist America’s travel assistance services.

Upon regular employment of 30 or more hours per week.

Dependent Life Insurance*

Optional term life insurance coverage for your dependents.

Upon regular employment of 30 or more hours per week.

Accidental Death and Dismemberment Insurance (AD & D)*

Optional insurance for accidental death and dismemberment. Available for the employee and family.

Upon regular employment of 30 or more hours per week.

Supplemental Life Insurance*

Optional additional life insurance for employee and family.

Upon regular employment of 30 or more hours per week.

Long Term Disability Plan

For disabilities lasting longer than 90 days:

Total disability pays 60 percent of pre-disability earnings.

Partial disability pays up to 70 percent of pre-disability earnings.

Earnings are integrated with other income.

Colonial Life Insurance*

Optional coverage for accidents, illness, cancer, and short-term disability income protection.

Upon regular employment of 20 or more hours per week.

Social Security Tax Exemption

Payment of Social Security taxes is optional. Employees who choose to opt out of paying Social Security taxes will receive an additional contribution to the pension plan.

Pension Plan

The company provides employees with a 5 percent contribution paid each pay period.

Profit Sharing Plan

The company provides a discretionary contribution, annually, based on profitability. The plan year is July 1 to June 30.

After 6 months of employment during a plan year and 1,000 hours of service. Participants must work 1,000 hours per plan year, be 18 years of age, and be employed on the last day of the plan year in order to receive a contribution.

Employees may contribute up to 80 percent of their pre-tax/after-tax earnings through voluntary payroll deduction. Fourteen investment options are available, as well as 5 life-style balanced funds.

Eligible immediately upon hire date. Must be 18 years of age.

Flexible Benefits Plan* (Cafeteria Plan)

Optional program allows employees to pay for certain employee benefits with pre-tax dollars instead of after-tax dollars; includes flexible spending accounts for medical reimbursement and dependent care reimbursement.

Incentive Bonus Program

Employees are eligible to receive up to $500 each quarter, based on the company’s ability to meet and exceed profit goals for said quarter.

After 6 months of employment. Bonuses are prorated based on hours worked during the quarter.

Long-Term Care Insurance*

Optional insurance plan that provides income in the event that an employee is unable to care for himself/herself.

Available to all regular employees, domestic partners, spouses, dependents, parents, and siblings.

Met Life Auto, Home, and Pet Insurance*

Optional insurance plan designed to insure the employee's home, car, and/or pets.

Child Care Center*

On-site subsidized child care center available for employee's, spouse's, or domestic partner's dependents ages 6 weeks to five years.  Open Monday through Friday, 6:30 a.m. until
6:00 p.m.

Upon regular employment with ARUP depending on availability.

UTA Transit Pass*

A UTA bus/TRAX pass (Eco pass) is available for 2008 for $68. There is a free shuttle service from the University Hospital to the main facility.

Available to all regular employees.

Other Employee Benefits
